4 stages of peripheral artery disease: From mild circulation issues to critical limb ischemia

Peripheral artery disease, or PAD, is a condition where the arteries become narrow or blocked over time, most often affecting the legs. Reduced blood flow means that oxygen and other essential nutrients struggle to reach the tissues of the body, for better movement, leading to pain, poor healing, and sometimes, tissue death. PAD develops gradually and may go unnoticed in its early stages, and sometimes, its symptoms can be misread for other potential diseases as well. Understanding its progression is crucial for early detection, effective management, and preventing serious complications. Its stages are as follows:

Asymptomatic: Mild circulation problems



In the first stages of PAD, symptoms are often subtle at first or completely absent from the picture. Many people may not even realise they have constricted or reduced blood flow in the arteries. Mild fatigue or slight cramping in the legs are the most overlooked symptoms of PAD (Early stages), which usually goes away with rest. At this stage, the arteries are beginning to narrow due to plaque buildup, but the blood supply is still sufficient for most daily activities. Early detection can make a big difference, routine check-ups, blood pressure tests and other doctor-recommended tests such as ankle-brachial index can help identify PAD before it progresses.

Intermittent claudication

As PAD progresses to stage two, symptoms become more noticeable. One of the most common signs is intermittent claudication. This is a basically leg pain, tightness or cramping that occurs during walking or exercise and eases with rest. The pain often affects our calve muscles, thighs or buttocks, can make walking longer distances increasingly difficult. At this stage, lifestyle modifications play a crucial role in slowing down the disease progression. The most recommended thing is to quit smoking (if you are a smoker), regular physical activity, maintaining a healthy diet or managing chronic conditions such as diabetes, high blood pressure can significantly reduce progression. Some patients may also require medication and consultation from doctors to improve blood flow and manage symptoms.

Critical limb ischemia


This stage represents the aggravating form of PAD, often referred to as critical limb ischemia or gangrene. In this stage, the blood flow to the legs gets limited, causing severe pain, even at rest. Infection risk gets high, tissues may begin to slowly die, leading to gangrene, if left untreated. Gangrene can result in the amputation of leg or life-threatening complications. Immediate medical intervention is necessary, and treatment options may include a bypass surgery, angioplasty or other procedures to restore the blood flow back to the affected limbs.

Acute limb ischemia

The fourth and last representation of PAD, a dangerous condition which when takes place, the blood flow to the legs are suddenly stopped. Unlike chronic conditions such as peripheral artery diseases, which develop gradually, ALI comes on quickly, often within hours or days. The sudden reduction in blood flow can cause severe pain, tissue damage, and, if not treated promptly, permanent disability or amputation.

Causes of acute limb ischemia:

Blood clots (thrombosis): A clot forming in an already narrowed artery can suddenly block blood flow.

Embolism: A clot or debris from the heart or another part of the body travels through the bloodstream and lodges in a limb artery. This is common in people with atrial fibrillation.

Trauma: Severe injury to a limb can damage blood vessels and interrupt circulation.

Vascular surgery complications: Rarely, surgery or medical procedures can accidentally lead to blocked arteries.
